There can be many different reasons for vaginal pain, and additional details are necessary to provide a proper answer. Some important questions include: the patient’s age, whether the pain is cyclic or persistent, the presence of pathological vaginal discharge, whether pain occurs during sexual intercourse (and if intercourse is possible at all), and whether the pain began spontaneously or followed a specific life event, such as childbirth.
Since the pain has already persisted for nine years, it is considered chronic. The main possible causes include vulvodynia, a condition often without clear findings. Endometriosis and interstitial cystitis are also potential explanations. After such a long duration, it is also reasonable to suspect pelvic floor muscle involvement, which can further worsen the pain.
I would advise consulting a specialist in chronic pelvic pain to exclude or confirm these conditions. In addition, consultation with a pelvic floor physiotherapist may significantly improve the patient’s quality of life.
